If you are considering the PDM Network, please click on the links below and download the "Credential Application" and "Personal Reference" forms. Fill them out and send them to our ministry address. Please send your "Credential Application" back to us and give one copy of the "Personal Reference" form to each of the two references you put on your application. Your references will fill their form out and send it back to us at the address below.
